Our goal is to make the referral process simple, secure, and responsive. Below you’ll find two easy ways to share patient information with us.
Step 1: Quick Notification (no patient info)
Use this form to notify our team of a referral. Please do not include patient names or identifiers. Once submitted, we’ll expect your referral form by fax or email.
Step 2: Complete the Referral Form (PDF)
Download and complete our secure referral form, then send it by fax to (888) 609-9756 or by secure email office@otteroralcare.com
Most referrals are processed within 1–3 business days, and our team will contact your patient directly to schedule.
